Proof of Birth Requirement for Daycare Waitlist Enrollment

"Be it resolved that, effective immediately, the following proof of birth requirement shall be implemented for individuals seeking to join the waitlist of Sunshine Learning Center:

Proof of Birth Requirement:

a. To be eligible for enrollment on the daycare waitlist, all parents or legal guardians must provide verifiable proof of the child's date of birth.

b. The daycare shall accept any form of proof of birth that is legally recognized, including but not limited to:

i. Birth certificates issued by relevant government authorities.

ii. Hospital records or birth records.

iii. Passport copies showing the child's date of birth.

iv. Official adoption certificates or documents.

v. Any other official documents recognized as valid proof of birth by local 

laws and regulations.


Submission Deadline:

a. Proof of birth documents must be submitted along with the daycare enrollment application or waitlist application.

b. Failure to provide the required proof of birth within the specified timeframe may result in the application being placed on hold or denied.


Verification Process:

a. The daycare staff shall be responsible for verifying the authenticity of the submitted proof of birth documents.

b. Any concerns or discrepancies regarding the submitted documents shall be addressed promptly with the applicant.

Waitlist Placement Date:

a. Applicants will be officially placed on the daycare waitlist on the date when all required documents, including proof of birth, are successfully submitted and verified.


Transparency and Fairness:

a. The daycare shall ensure that this proof of birth requirement is communicated clearly to all potential applicants through various channels, including the daycare's website, application forms, and informational materials.

b. The daycare shall apply this requirement consistently and fairly to all applicants without discrimination.

This motion was adopted to enhance the transparency and integrity of the daycare enrollment process and to ensure that accurate information about children's ages is available to assist with class placement and scheduling.
